How do we count LDL level?

According to Friedewald the low-density lipoprotein level is: 

LDL = total cholesterol – HDL - (TG/5)

All concentrations are given in mg/dl.

The quotient (TG/5) is used to estimate the VLDL (Very Low Density Lipoprotein) level.

The Friedewald equation limitations

This equation should not be used when: 

  • high level of chylomicrons is present
  • plasma triglyceride concentration exceeds 400 mg/dl
  • dysbetalipoproteinemia (type III hyperlipoproteinemia) is present.

In these circumstances LDL level should be measured directly.

How to interpret the obtained value?

Check your level of LDL against the chart below to interpret results.

 <100Optimal LDL cholesterol, corresponding to a reduced risk of heart disease
 100 - 129
Near optimal LDL level
 130 - 159Borderline high LDL level
 160 - 189
High LDL level
 >190Very high LDL level, corresponding to a highly increased risk of heart disease

A few practical remarks

If your LDL level is too high try to modify your lifestyle and composition of your meals. If this approach does not work, ask your doctor for help.
